All patients must complete their information before seeing the doctor. If we do not participate with your insurance plan, full payment is due at the time of service. We accept cash, checks, Visa, MasterCard, and Discover. We will submit a claim to your insurance carrier on your behalf, and your carrier will reimburse you directly. In the event your insurance coverage changes, it is your responsibility to notify us and provide your updated card and information.
We will do our best to provide a rough estimate of any potential out-of-pocket costs for each upcoming visit based on your individual treatment plan.
Usual and Customary Rates
Our practice is committed to providing the best treatment for our patients, and we charge what is usual and customary for our area. You are responsible for payment regardless of any insurance company’s arbitrary determination of usual and customary rates.
Outstanding balances are discouraged and must be cleared before the next appointment for any account member or within 30 days of treatment, whichever comes first. Appointments for non-emergency treatment may need to be postponed until outstanding balances are paid.
Delinquent balances over 60 days old will be referred to a collection agency. All referred accounts are marked “inactive.” If your account is assigned to a collection agency, you will be responsible for a 25% collection fee, 18% interest, court costs, and attorney fees, as allowed by law.
Minor Patients
The adult, parent, or guardian accompanying a minor is responsible for full payment at the time of service and for any future treatment. This policy applies regardless of divorce, separation, or court-mandated designation of financial responsibility. We are not a party to any such court order, and our financial relationship is established with you.
Miscellaneous
A $75.00 fee will be charged for missed appointments without at least 24 hours’ notice.
A $35.00 fee will be applied to all checks returned unpaid by the bank.
